The speech-information recoding/reproducing means is sandwiched from both sides by a part of the regions, facing the speech-information recording/reproducing means, of the t wo support sheets. A region of the speech-information recording/reproducing means, where at least a speech information output section is incorporated between the two surface sheets, is foldable over the other region along the folding line.
