The device includes: a wiring substrate having a first opening portion therein and including: a substrate having an upper surface and a lower surface opposite to the upper surface; an upper side wiring layer formed on the upper surface of the substrate, and a lower side wiring layer formed on the lower surface of the substrate, an optical waveguide formed on the lower side wiring layer; a first optical element connected to the upper side wiring layer; a first circuit element electrically connected to the first optical element through the upper side wiring layer; a second optical element connected to the lower side wiring layer through the first opening portion; and a second circuit element electrically connected to the second optical element through the lower side wiring layer.
