    Abstract: A single ultrasonic surgical apparatus can provide a substantial cutting effect on tissue, a substantial coagulation effect on tissue, and an appropriate blend of simultaneous cutting and coagulation effects on tissue. The ultrasonic surgical apparatus is comprised of a handpiece which incorporates a transducer and a surgical tool, a source of electrical energy, a controller, and a switch. The apparatus may be operated at two frequencies within the ultrasonic spectrum, where the first frequency is selected at the low end of the spectrum for enhanced tissue cutting performance, and the second frequency is approximately three times higher for enhanced tissue coagulation performance. The two frequencies may be operated selectively or concurrently, and may be independently adjustable as to amplitude. The transducer may be composed of magnetostrictive or piezoelectric elements. The surgeon to set the desired amplitude of the mechanical vibrations. The transducer is mounted within the handpiece at a location that corresponds to a vibration node common to both the first frequency and the second frequency. Also claimed is a method for use, including the steps of: developing an ultrasonic resonance at a first frequency; developing an ultrasonic resonance at a second frequency which is approximately three times greater than the first frequency; resonating an ultrasonic transducer at the first frequency concurrently with the second frequency; and supporting the transducer where the nodes of vibration at the first frequency are substantially coincident with the nodes of vibration at the second frequency.

    Abstract: An apparatus for transcervical sterilization or transcatheter embolization with a controlled bipolar RF catheter for creating thermal lesions in the fallopian tubes or thrombosing vessels has a catheter elongate along an axis thereof with a patient end. The catheter is circular and sized for transcervical insertion into the fallopian tube or transcatheter vessel insertion, respectively. A connector on an end of the catheter opposite the patient end has the terminations for RF and monitoring and is shaped for the surgeon to manipulate during placement and withdrawal. Two or more bipolar electrodes on the patient end are placed so each electrode is spaced from another with each circumscribing the catheter. A mucosa or thrombus sensor responsive to applied RF energy passing between the two or more bipolar electrodes determines the condition of the transmural formation of a lesion or the thrombus between each of the electrodes. The sensor is a temperature sensor positioned in the space between the electrodes measures the change in mucosal layer temperature or the thrombus during the application of RF energy. An RF generator electrically coupled to the electrodes and the sensor is alternately a phase detector in circuit with the coupling determines reactance of tissue as an indicator of lag or lead of the voltage wave form or current wave form delivered or is an impedance responsive circuit positioned in the RF generator to determine voltage and current delivered so a calculator finds impedance delivered for finding initial electrode contact with the tissue and for measuring the change in mucosal layer. The temperature sensor connects to an RF generator with control circuitry regulating delivery to a temperature range. A method for sterilization has the steps of inserting a catheter, delivering RF and monitoring the effect on the mucosa or thrombus, circumscribing the catheter with two or more bipolar electrodes, each of the electrodes spaced from one another and passing RF energy therebetween.

    Abstract: The instrument includes an extended rigid shaft (14) with a proximal end (15) and a distal end (16), an inner chamber and an outer diameter of 10 mm or less. Also at least three electrodes (17), are arranged movably in the inner chamber. At least one of the electrodes is connected with the first output connection, and at least one other electrode is connected with the second output connection. An operating system (19) is fitted in vicinity of the proximal end (15) of the shaft (14), and is mechanically connected with the electrodes (17), in order that the electrodes travel out and back in relation to the distal end (16). An electric switch makes an electrical circuit through the electrodes and between the first and second output connections.

    Abstract: A replaceable accessory and cord set (10) for an electrosurgical instrument (11) that has a proximal end (15) handle (26) for holding by the surgeon and distal end (16) effectors for delivery of electrosurgery to a patient (12). A cord (17)electrically couples an electrosurgical generator (14) to the instrument with two or more conductors (18). A handswitch (19) with normally open contacts (20) is near the proximal end (15). One of the contacts (20) attaches to an active conductor and another of the conductors connected to the other contact (21). A receptacle (22) on the handswitch (19) connects electrically to the contact (20). A terminal (23) on the electrosurgical instrument (11) conjugates with the receptacle (22) in electrical contact (20) with the active conductor. An insulated support (24) on the handswitch (19) engages the instrument preventing relative movement. A button on the support (24) is accessible to the surgeon, but remote from the receptacle (22) and the terminal (23). The button is electrically isolated from the contacts (20), the receptacle (22), the terminal (23) and the conductors. The button moves on the support (24) for closing the contacts (20) and is positioned for control by the surgeon's finger. The handle (26) is insulated and the effectors are located at the distal end (16) and could include scissors (28), graspers or hooks. The effectors are in a bipolar circuit with a return on one effector connected to one of the return conductors (18''') for completing the circuit. The effectors when in a monopolar circuit have a rerturn pad (29) on the patient (12) as a part of the circuit. The support (24) electrically insulates the button and the handswitch (19) preventing leakage to the surgeon. The generator connects to the return conductor (18") coupled to one of the effectors for return of energy. The support (24) may have a pressure sensitive adhesive (30) thereon to secure the button on the handle (26). A plug (31) removeably connects the conductors to thegenerator. The handswitch (19) controls delivery a coagulating wave form. The instrument is elongate between the handle (26) and the effectors so that laparoscopic electrosurgery may be performed.
