    Abstract: An electrosurgical generator has an output power control system (10) that causes the impedance of tissue (14) to rise and fall in a cyclic pattern until the tissue ( 14) is desiccated. The advantage of the power control system (10) is that thermal spread and charring a re reduced. In addition, the power control system (10) offers improved performance for electros urgical vessel sealing and tissue welding. The output power is applied cyclically by a control system w ith tissue impedance feedback. The impedance of the tissue (14), until the tissue becomes fully desic cated. High power is applied to cause the tissue (14) to reach a high impedance, and then the power i s reduced to allow the impedance to fall. Thermal energy is allowed to dissipate during the low power c ycle. The control system is adaptive to tissue in the sense that output power is modulated in resp onse to the impedance of the tissue.

    Abstract: A circuit, for monitoring and controlling parameters of an electrosurgical unit, ESU, relative to load and the RF energy, has a load responsive output sensing circuit that measures the ESU load. A signal modifier, attached to the sensing circuit enhances the signals measured and transmits them to a buffer. An analog to digital converter, A/D, digitize the signals and samples wave pulse train at about eight million samples per second. A data memory stores the digitized signals. A RF drive clock connects to the ESU output; a sample clock uses phase shifting to interrogate the input signals to a processor, DSP, at a greater sampling rate of frequency than without. The DSP receives the stored signals from the data memory and processes them while monitoring and calculating ESU parameters measured, i.e. voltage, current, power, load impedance, leakage current, peak to peak voltage, peak to peak current, spectral content and/or crest factor of the RF wave pulse train energy to use as controlling feedback to the either a high voltage power supply in the ESU, regulating the RF drive pulses or both. A method monitors and controls the ESU relative to load has the steps of collecting parameters with the ESU output sensing circuit responsive to loads; enhancing signals with the signal modifier; transmitting signals to the buffer; converting signals with the A/D converter; storing signals in the data memory; receiving signals in the DSP, and processing, monitoring and controlling signals by repeatedly measuring ESU output parameters.

    Abstract: An apparatus (20) for transcervical sterilization or transcatheter embolizat ion with a controlled bipolar RF catheter (21) for creating thermal lesions (22) in t he fallopian tubes (23) or thrombosing vessels has a catheter (21) elongate along an axis there of with a patient end (24). The catheter (21) is circular and sized for transcervical insertio n into the fallopian tube or transcatheter vessel insertion, respectively. A connector (25) on an end of the catheter (21) opposite the patient end (24) has the terminations (26) for RF and monitoring and is shaped for the surgeon to manipulate during placement and withdrawal. Two or more bipolar electrodes (29, 30 and 31)on the patient end (24) are pl aced so each electrode is spaced from another with each circumscribing the catheter (21). A mucosa or thrombus sensor responsive to applied RF energy passing between the two or m ore bipolar electrodes (29, 30 and 31)determines the condition of the transmural formati on of a lesion or the thrombus between each of the electrodes. The sensor is a temperature sensor positioned in the space (34) between the electrodes measures the change in m ucosal layer temperature or the thrombus during the application of RF energy. An RF gener ator (27) electrically coupled to the electrodes and the sensor is alternately a phase detector(35) in circuit with the coupling determines reactance of tissue as an indicator of lag or lead of the voltage wave form or current wave form delivered or is an impedance responsi ve circuit (39) positioned in the RF generator (27) to determine voltage and current de livered so a calculator finds impedance delivered for finding initial electrode contact w ith the tissue and for measuring the change in mucosal layer. A blunt tip at the patient end (2 4) facilitates introduction into the tube or vessel to avoid perforation. The temperature s ensor connects to an RF generator (27) with control circuitry regulating delivery to a temp erature range. A proportional controller regulates the RF energy delivery to control temper ature rise time and to maintain the temperature near the mid point of the temperature range. A method for sterilization has the steps of inserting a catheter (21), delivering RF and monitoring the effect on the mucosa or thrombus, circumscribing the catheter (21) with two or more bipolar electrodes, each of the electrodes spaced from one another and passi ng RF energy therebetween.

    Abstract: A single ultrasonic surgical apparatus can provide a substantial cutting effect on tissue, a substantial coagulation effect on tissue, and an appropriate blend of simultaneous cutting and coagulation effects on tissue. The ultrasonic surgical apparatus is comprised of a handpiece which incorporates a transducer and a surgical tool, a source of electrical energy, a controller, and a switch. The apparatus may be operated at two frequencies within the ultrasonic spectrum, where the first frequency is selected at the low end of the spectrum for enhanced tissue cutting performance, and the second frequency is approximately three times higher for enhanced tissue coagulation performance. The two frequencies may be operated selectively or concurrently, and may be independently adjustable as to amplitude. The transducer may be composed of magnetostrictive or piezoelectric elements. The surgeon to set the desired amplitude of the mechanical vibrations. The transducer is mounted within the handpiece at a location that corresponds to a vibration node common to both the first frequency and the second frequency. Also claimed is a method for use, including the steps of: developing an ultrasonic resonance at a first frequency; developing an ultrasonic resonance at a second frequency which is approximately three times greater than the first frequency; resonating an ultrasonic transducer at the first frequency concurrently with the second frequency; and supporting the transducer where the nodes of vibration at the first frequency are substantially coincident with the nodes of vibration at the second frequency.
