Abstract: Pharmaceutical compositions comprising aliphatic alcohols are disclosed for use in therapy, for example eye therapy and in particular for the treatment of discomfort and visual artifacts following laser surgical procedures on the cornea of the eye. Such compositions reduce the inflammation associated with photorefractive keratectomy and promote the regrowth of epithelial tissue over the reprofiled corneal surface. The compositions may comprise aliphatic alcohols having a chain length ranging from about C-20 to about C-26, and may include docosanol, tetracosanol or hexacosanol, or mixtures thereof.

The corneal topographer specifically shown comprises a rigid reference member having a reference surface of a predetermined shape applied to the surface of the eye. The shape of the reference surface is correlated with the measured information to determine the shape of the corneal surface. Several specific designs are shown that use optical, acoustic, acousto-optic or capacitive techniques to determine a distance between the reference surface and the corneal surface over a multiplicity of data points sufficient in number and spacing to represent the local topography of the surface of the eye.

Abstract: Disclosed is a masking apparatus for use in laser reprofiling of corneal tissue. The masking apparatus comprises a support structure, which can be affixed upon a cornea, and a mask connected to the support structure and disposed above the cornea. The mask includes a light restriction element, which may be an iris or a series of apertures in a rotatable plate, and a control element for regulating laser impingement on the corneal tissue. In operation, the light restriction element is adjusted to regulate the radiant energy incident upon the cornea. The apparatus accordingly provides a method for the selective photoablation of the cornea.

Abstract: -21- Methods and systems are disclosed for corneal reprofiling employing an annular beam of ablative radiation to correct refractive errors in vision, and/or create "blend zones" in order to reduce any surface discontinuities on the cornea following hyperopia-correcting or astigmatism-correcting procedures. Preferably, the methods and systems of the present invention also operate to change the intensity distribution of the light beam, such that the annulus of radiation has a varying intensity as a function of radius and, thereby, achieves a blending or other desired effect with the least number of pulses. Methods of photorefractive keratectomy (PRK) are disclosed, includin g the steps of (a) selectively reprofiling a cornea of an eye by removing a volume of corneal tissue with ablative radiation to form a reprofiled region in the optical zone whic h steepens, at least in part, the central curvature of the cornea; and (b) applying to a peripheral region of the cornea an annular beam of ablative radiation having an intensity which varies radially from a inner to a outer segment of the annular beam to create a blend zone, thereby providing a smooth transition between the repro filed region and an unablate d region of the cornea. Optical lens elements also are disclosed having a generally coni cfront face and a rear face which is generally perpendicular to the path of the incident bea m. Light passing through this optical element will be refracted into an annular beam. Variations in the intensity profile of the annular beam can be introduced by employing an initial beam which has a non-uniform profile, or by modifying the shape of either the fro nt face or the rear face of the optical element.

Abstract: Methods and apparatus for corneal reprofiling are disclosed in which a beam-swapping assembly is employed to redirect rays of ablative radiation passing through the assembly. Essentially, the beam-swapping assembly turns the beam profile "inside out" redirecting the inner most rays to the periphery of the beam while redirecting the outer rays at the same time to the center of the beam. The beam-swapping assembly can be used in conjunction with a beam-shaper, such as an adjustable iris or other beam-shaping device, to create convoluted annular ablation patterns of variable depth and, thereby, perform hyperopia and/or astigmatism correcting procedures.
