Abstract: A surgical table includes a sagittal adjustment device for manipulating the position of a patient. The surgical table includes a base portion including an upper surface for spacing the sagittal adjustment device from the ground. The sagittal adjustment device includes a first support portion and a second support portion. The first and second support portions are supported by and moveable over the upper surface of the base portion. The first and second support portions each include an upper surface configured to support portions of the body of the patient thereon. One of the first and second support portions is pivotally attached to the base portion, and moveable between a first position and a second position. The pivotal movement between the first and second positions of the one of the first and second portions serves in repositioning the body of patient to manipulate the spine of the patient.

Abstract: An approach is provided for delivering therapeutic materials to an intervertebral disc via a sub-ligamentous space. The approach includes positioning a tool at an interface of a longitudinal ligament and an outer surface of the intervertebral disc, in which the interface is the sub-ligamentous space. The tool may include a first needle and a second needle housed within the first needle. An insertion end of the first needle may include a shallow beveled end. The approach includes inserting the insertion end of the first needle into the sub-ligamentous space. The approach includes deploying the second needle from within the first needle into at least one of an annulus and a nucleus of the intervertebral disc. The approach includes delivering the therapeutic materials to the at least one of the annulus and the nucleus.

Abstract: An assembly for remotely manipulating a patient's spine. A compressor/distractor mechanism attaches to spinal anatomy or vertebral screws on adjacent vertebrae at a spinal surgical site on a patient's spine. A flexible cable is operatively connected to the compressor/distractor mechanism and to a control mechanism. The control mechanism manipulates the flexible cable, controlling the compressor/distractor mechanism to compress or distract the vertebrae. A rotatable surgical table with a remote control can be used in place of the compressor/distractor mechanism and the control mechanism. The rotatable surgical table can rotate the patient to place the spinal surgical site in a desired compressed or distracted position, and to place the patient in a desired orientation with respect to a horizontal reference plane. The assembly permits manipulation of the spine at the surgical site via a posterior approach, while the surgeon can also manipulate the spine at the surgical site via an anterior, lateral or oblique approach.

Abstract: An expandable spinal implant includes a distal projection extending from only one side of the implant, ending in an anterior tip, the anterior portion and anterior tip defining an elongated distal end hook, which is wider than the proximal end. The distal end hook rotates around the spinal cord, aligning the implant with a desired pathway, then inserts into place in the disc space between the vertebrae. The elongated widened distal end hook provides a TLIF approach, distributes loads, provides anterior rim engagement, and creates lordosis.

Abstract: A pivoting wedge expandable spinal implant. An upper portion and a lower portion are pivotally connected together. The implant, in a collapsed position, is inserted into a disc space. A driving screw engages and applies a force to a pushing portion, driving the pushing portion toward the implant's distal end. The pushing portion engages and drives a wedge toward the implant's distal end. The wedge pivots upward against an inner surface of the lower portion. The wedge continues to pivot along an inner surface of the upper portion, translating the force to the upper portion, pivoting and expanding the upper portion to an expanded position.

Abstract: An intervertebral implant comprises a first component and a second component. The second component includes an actuator and a third component comprises a first ramp and a second ramp axially spaced apart from the first ramp. The third component comprises rails including at least a portion of the ramps. At least one of the ramp portions disposed with the first rail has a first height and at least one of the ramp portions disposed with the second rail has a second height. The first height is greater than the second height. The actuator is engageable with the third component to effect axial translation such that the ramps engage at least one of the components between a first configuration and a second configuration. Methods of use are disclosed.
