Abstract: An ultrasonic clamp coagulator assembly that is configured to permit selective cutting, coagulation and clamping of tissue during surgical procedures. An elongated portion of the instrument can be configured for endoscopic applications and has an outside diameter of less than 6 mm. The construction includes a clamping mechanism, including a clamp arm pivotally mounted at the distal portion of the instrument, which is specifically configured to create a desired level of tissue clamping forces, exceeding 4 pounds when the trigger is fully closed. The clamping mechanism includes a force-limiting mechanism that effectively smooths out abusive tissue forces.

Abstract: An ultrasonic instrument comprises a body, a shaft assembly, an ultrasonic blade, and a pivoting member. The shaft assembly extends distally from the body. The ultrasonic blade is positioned distal to the shaft assembly. The pivoting member is pivotable with respect to the blade from an open position to a closed position to thereby clamp tissue between the pivoting member and the blade. One or both of the shaft assembly or the pivoting member comprise a guide feature. The guide feature is configured to provide lateral alignment of the distal portion of the pivoting member with the blade as the pivoting member is pivoted to the closed position.

Abstract: An ultrasonic clamp coagulator apparatus is disclosed including a housing including an actuator, an outer tube, an ultrasonic waveguide, an ultrasonically actuated blade, a clamp member pivotable with respect to the blade, and a force limiting mechanism including a first compression spring having a first spring constant, positioned between the actuator and the clamp arm, and a second compression spring having a second spring constant less than the first spring constant. The first compression spring transfers a force at a first rate as a function of a motion of the actuator to pivotably move the clamp member toward the blade. The second compression spring transfers a force as a function of the tissue thickness positioned between the clamp member and the blade. At least one of the first and second compression springs is pre-compressed to provide a preload less than the respective spring constant.

Abstract: Systems, devices, and methods are operable to track usage of a surgical instrument and modify the performance of the surgical instrument based on the prior usage of the surgical instrument. Some surgical instruments are designed to have a limited service life beginning at their first use, or a limit to their overall usage in order to ensure safe use of the sensitive instruments. However, a lack of ability to track usage characteristics when the instrument is separated from an external power supply allows for user abuse and avoidance of such safety mechanisms. Adding a battery or capacitor to the instrument may allow for an ability to track usage when the instrument is separated from an external power supply. Implementing special user prompts, device use ratios, and device use half-life upon powering down of an instrument may additionally be used to prevent circumvention of safety features.

Abstract: An apparatus includes a shaft assembly, an ultrasonic blade, and a clamp assembly. The shaft assembly includes an acoustic waveguide operable to transmit ultrasonic vibrations to the blade. The clamp assembly includes a clamp arm pivotable toward and away from the blade about a pivot axis, to clamp tissue between the clamp arm and the blade. A rotation feature may provide rotation of the blade relative to the clamp arm about the longitudinal axis of the waveguide. Alternatively, the rotation feature may provide rotation of the clamp arm relative to the blade about the longitudinal axis. The rotation feature may be driven based on pivotal positioning of the clamp arm relative to the blade about the pivot axis. The rotation feature may selectively lock and unlock the angular position of either the blade or the clamp arm about the longitudinal axis at any of a number of predetermined angular positions.

Abstract: Various aspects of a generator, ultrasonic device, and method for estimating and controlling a state of an end effector of an ultrasonic device are disclsoed. The ultrasonic device includes an electromechanical ultrasonic system defined by a predetermined resonant frequency, including an ultrasonic transducer coupled to an ultrasonic blade. A control circuit measures a complex impedance of an ultrasonic transducer, wherein the complex impedance is defined as Z g ( t ) = V g ( t ) I g ( t ) . The control circuit receives a complex impedance measurement data point and compares the complex impedance measurement data point to a data point in a reference complex impedance characteristic pattern. The control circuit then classifies the complex impedance measurement data point based on a result of the comparison analysis and assigns a state or condition of the end effector based on the result of the comparison analysis. The control circuit estimates the state of the end effector of the ultrasonic device and controls the state of the end effector of the ultrasonic device based on the estimated state.

Abstract: Various systems and methods for determining the state of an ultrasonic electromechanical system are disclosed. A control circuit can be configured to monitor the change in resonant frequency of an ultrasonic electromechanical system of the ultrasonic surgical instrument as the ultrasonic blade oscillates and determine the state or change in state of the ultrasonic electromechanical system accordingly. The change in state of the ultrasonic electromechanical system can include, for example, the change in temperature of the system. In some aspects, the control circuit can be configured to modify the operation of the ultrasonic electromechanical system or other operational parameters of the ultrasonic surgical instrument according to the state or change in state of the system.

Abstract: Various forms are directed to systems and methods for dissection and coagulation of tissue. A surgical instrument includes an end effector configured to dissect and seal tissue at a distal end thereof, and a selector switch having a plurality of surgical modes. A generator is electrically coupled to the surgical instrument and is configured to deliver energy to the end effector. Each surgical mode of the selector switch corresponds to an algorithm for controlling the power delivered from the generator to the end effector, and each algorithm corresponding to the plurality of surgical modes is configured to allow a user to control the power output level of the generator.
