    Abstract: Embodiments of the invention relate to an assembly for forming a cased well. The assembly may include an undercut drillable cementing shoe with a casing stri ng connection at a first end. The shoe includes external tubing that forms a second end of the shoe and has a first section defining an enlarged inner diameter relative to a second section of the external tubing. The assembly may also include an earth removal member coupled to the second end of the shoe. The first section of the shoe is disposed between the earth removal member and the second section of the shoe. In another embodiment, the assembly may include a casing string having a first portion with a larger inner diameter than a second portion. T he assembly may also include an earth removal member coupled to an end of the casing string. The first portion of the casing string is disposed between the earth removal member and the second portion of the casing string.

    Abstract: A method for connecting or removing a tubular 122 from the top tubular 124 of a tubular string without interrupting drilling or the circulation of fluid through the tubular string. The method comprises the steps of independently operating a rotary drive 130 and a top drive 120 to continuously provide rotational and axial movement of the string in the wellbore 105 and threadably connecting or disconnecting the tubulars by operating both drives at different relative speeds. The threaded ends of the tubulars are mated within a fluid circulating device 140 so as to circulate the fluid continuously through the string either from the circulation device 140 when the tubulars are disconnected or through the uppermost tubular 122 whilst the tubulars are connected. Also disclosed is a rotary drive mechanism (figure 3) comprising an hydraulically operated axial displacement piston (fig.3, 300) and a slip (fig.3, 132) assembly for preventing rotational and axial movement between the piston and the tubular 124 therein.
