Abstract: An alignment plate for mounting of a structure 22 to a foundation 30 featuring a plate with a plurality of first mounting slots 114a/116a/116b aligned with a length of the plate and a plurality of second mounting slots perpendicular with the first slots and aligned with a width of the plate. The plate may be connected to a foundation by threaded bars connected to the first mounting slots and connected to a structure by threaded bars connected to the second mounting slots. The slots may be offset to one side of the plate to provide a greater ability to connect misaligned structures and foundations. The plate may include a flange 104 on one end and may be used to assist in affixing a communication tower to a foundation.
