    Abstract: An implantable electrically actuated medical device having one or more electrochemical cells and operative electric circuitry mounted in a metallic container, all of which are positioned in a spacer member made of a material which is highly permeable with respect to hydrogen gas, relatively impervious to liquids, and bonds well with epoxy resin. The device is encapsulated with an epoxy resin which is relatively impermeable to hydrogen gas and is impermeable to body fluids. The device incorporates an additional path or means which is highly permeable to hydrogen gas and impervious to fluids which may be formed integral with the spacer and be in the form of pins positioned around the same to position the spacer with the components therein during encapsulation and which extend through the encapsulation. The path may also be in the form of a ridge around a recess in the spacer mounting the electronic components to strengthen the same and provide the path for hydrogen gas.

    Abstract: An implantable medical device has a medical unit, such as a pacemaker lead, and a casing at least partially enclosing the medical unit. The casing is formed of a base polymer such as a polyurethane, a polyurethane copolymer, a fluoropolymer and a polyolefin or a silicone rubber. The casing has biologically active agents covalently bonded to the base polymer. The biologically active agents can be attached to the base polymer as surface active end groups. As an alternative, the biologically active may be attache d to a backbone the base polymer. As yet a further alternative, the biological ly active agents may be attached to surface modifying end groups, which are in turn attached to the base polymer. Examples of suitable biologically active agents are microbial peptide agents, detergents, non-steroidal anti- inflammatory drugs, cations, amine-containing organosilicones, diphosphonate s, fatty acids and fatty acid salts.

    Abstract: The present invention provides delivery systems for delivering conduction protein genetic material to cardiac cells in localized areas of the heart to improve the conductance therein. More specifically, there is provided a system for delivering connexin proteins or nucleic acid molecules encoding connexin proteins to a site in the heart which has been determined by mapping procedures to have a conduction disturbance. For cases where conduction is impaired, selected genetic material is delivered by Applicants' delivery system to cells around the disturbance area, in order to enhance overall conductivity patterns; in other cases, genetic material is selected to slow conduction in affected areas, so as to prevent, e.g., brady-tachy syndrome.

    Abstract: A cardiac pacing lead or other stimulation lead carrying a steroid compounded within a polymer matrix. The steroid is dispersed in thermoplastic or thermoset polyurethane or polyurea forming monomers or prepolymers. The mixture is then chain extended by addition of the appropriate difunctional or multifunctional isocyanate, hydroxyl or amine curing agent to form a solid polymer. The resulting formulation is molded or cast into a desired shape, and incorporated in the distal end of a medical electrical lead, adjacent to or within the electrode. In a preferred embodiment, the medical lead employs an extendable helix which is screwed into body tissue, and the polymer containing the drug takes the form of a generally dimensionally stable annular member mounted to the lead, encircling the helix.

    Abstract: A medical electrical lead system having a torque transfer stylet. In a preferred embodiment, the torque transfer stylet comprises an elongated, stylet wire having proximal, intermediate and distal sections. The proximal and distal section of the stylet wire have a first diameter. The intermediate section spans between the proximal and distal sections. The stylet wire in this region, however, has a second diameter, the second diameter smaller than the first diameter of the proximal and distal sections. In addition, the intermediate section further has a length of torque coil wrapped thereabout. The torque coil preferably is wound in a coiled diameter which is the same as the first diameter of the proximal and distal sections of the stylet wire. Through such a configuration the stylet has an overall nearly uniformed diameter through its length.

    Abstract: An improved electrode for establishing electrical contact with a nerve of a patient. This electrical contact may be used to sense neural activity on the nerve or to artificially stimulate it to perform various medical treatments. The electrode has an outer cuff of an insulating material which is sutured around the nerve to be contacted. A drug impregnated layer of material is positioned inside of the outer cuff. This material is a polymeric matrix which permits the drug to leach out at a predetermined rate. The drug is a steroid such as dexamethasone sodium phosphate. The actual electrical contact is produced by metallic foil which is positioned on the surface of the drug impregnated layer of material which is located inside of the cuff. An insulated lead electrically couples the metallic foil to an electronic circuit located remote from the nerve. The leaching out of the drug serves to control irritation, swelling, and impedance of the nerve/electrode interface.

    Abstract: A single-pass body implantable lead for transvenous insertion through a single vein to simultaneously sense and/or stimulate the tissue of both the right atrium and right ventricle. The lead employs a single bifurcated connector at the proximal end. Insulated conductors for the ventricle and atrium are branched from a single outer sheath. The ventricular and atrial branches are established in fixed relation to one another. The ventricular branch is of sufficient length that it has excess length for the largest heart. The electrode of the ventricular branch is held within the right ventricular apex by tines. The atrial branch has a "J" shape imparted to the conductor coil. The atrial electrode is maintained in position within the right atrial appendage by tines. The ventricular branch is sufficiently flexible such that any excess length is merely coiled in some convenient position. The distal portion of the ventricular branch is made slightly more stiff than the proximal portion causing the excess length to loop within the atrium. Stylets are used to guide insertion.

    Abstract: A body implantable lead of the type having a lead body, at least one electrode carried by the lead body and tissue engaging tines extending at an acute angle front the lead body at a location adjacent the electrode. The present invention correlates the tensile stiffness of the lead body with the stiffness of the tines to optimize the tine holding force. The tines provide an adequate holding force to minimize unintentional lead dislodgements without requiring an excessive force for intentional dislodgement of the lead for repositioning. Tine length and the angle at which they extend from the lead body are also discussed as affecting the probability of tine engagement with cardiac structure as well as having an effect on the holding force. In a preferred embodiment, the lead body has a spring constant (as defined herein) of 2.0 lb-in./10 in. or less and a tine stiffness (also defined herein) of less than 5x10-3 lb-in.2. The tines are from two to six millimeters long and extend from the lead body at an angle from 30.degree. to 80.degree.. More preferably, the tine stiffness is 3x10-3 lb-in.2 or less with the tines being from four to six millimeters long and forming an angle with the lead body from 45.degree. to 60.degree.. In the preferred embodiments, the exposed surface of the lead body and the tines are formed of a body compatible urethane. In the most preferred embodiment, the tine stiffness is approximately 2.5x10-3 lb-in.2.
