Abstract: A clamping force mechanism (10) and its method of use with electrosurgery allow a user to seal and/or joint patient's particular vascular tissue; the mechanism is elongate with user and patient ends (14). An actuator (15) is a t the user end (13) and the effectors are at the patient end (14). Each effect or has a face of an area to contact the particular vascular tissue. A lost moti on connection (18) transfers user actuation to the effectors to hold a predetermined clamping force during electrosurgical tissue sealing. A yieldi ng member (19) in the loss motion connection clamps the particular tissue betwe en the faces with a predetermined force. The yielding member (19) is a spring (20), slip clutch (36) or hydraulic coupling possibly near the actuator (15) . An active electrode is carried on one end effector and a return electrode (2 4) contacts the tissue so an electrosurgical energy supply (25) connected thereacross delivers energy therebetween. A feedback circuit (27) responds t o parameters of energy delivered to tissue. A temperature sensor (28) on one face and an impedance monitor (29) respond to energy delivered. A control (3 0) applies energy to held tissue to seal and/or join it. The method transfers user actuation of the effectors with the lost motion connection (18), maintains clampingforce while sealing and applying electrosurgical energy to seal and/or join held tissue. Energy is applied to a temperature or an impedance. The method shifts the yielding member (19) so the opposing faces (17) clamp the particular tissue therebetween with the force from the spring (20), via a transfer rod or by the hydraulic coupling or the slip clutch (36 ) for a range of pressure.

Abstract: The rinsing device (10) is used in conjunction with a suction tube (11) and a container holding the rinsing fluid. The device further comprises a funnel section (12) with a widened inlet (15) at one end connected via a channel (17) to a narrow outlet (16). In a chamber (18) within the funnel section is a projecting piece (19) which acts as a valve to close the inlet from the outlet. A port (20) is located in the projecting piece and a cover (28) is arranged proximally to it over the inlet of the funnel. The cover has an inlet for the rinsing medium and a channel (31) for the suction tube. A lip type valve assembly in the projecting piece acts to control fluid flow through the device. The device is assembled in stages from discreet components.

Abstract: A valve and irrigator 10 used with a suction tube 11 has an elongate funnel 12 with an axis through an enlarged entrance 15 and an opposed extended exit duct 16 of a flexible elastomer. A flow passage 17 through the elongate funnel 12 connects the entrance to the exit permitting fluid communication. A web 19 across and near the enlarged entrance 15 sealingly separates that from the exit. The web 19 and the elongate funnel 12 are made of silicone elastomer and are adhesively adhered to together. A portal 20 in the web 19 passes therethrough along the axis and is circular in shape to surround in fluid tight engagement the suction tube 11. A lid 28, proximally of the web 19 on the enlarged entrance 15, has an inlet 29 for irrigation and a pass through 30 for the tube; the lid 28 closes the enlarged entrance 15 forming a chamber 31 between the web 19 and the lid 28. A slit 23 within the web 19, near the portal 20 and spaced from the axis is in the form of a closed mouth 24 composed of a pair of sealing lips. Normally the pair of lips 25 are together but are able to separate and define a passageway 26 for fluid flow through the web 19. A peripheral bump 27 is radially disposed relative to the slit 23 and is located on the entrance in position to align flexure force therethrough and toward the axis. A method of using the valve and irrigator 10 with the suction tube 11 has steps including locating an elongate funnel 12 along the axis with the entrance thereon. Positioning the exit on the elongate funnel 12 opposite the entrance and along the axis is a step. Merging the exit with the entrance is step. A step is permitting fluid communication through the flow passage 17 between the entrance and exit. Sealingly separating the entrance and exit with the web 19 is a step. The step of passing the portal 20 through the web 19 along the axis so it surrounds tube therethrough. A method for making the valve and irrigator 10 has steps of locating the web 19 near the entrance, passing through the web 19 the portal 20 and slit 23.
