Abstract: Blunt tissue dissection using a sharp cutting instrument to make an initial cut in tissue and an inflatable device to further separate tissue layers on either side of the cut. The sharp cutting instrument may be advanced through the lumen of an elongate delivery member such as through a working channel of a medical scope. The inflatable device is inserted over the exterior of the elongate delivery member.

Abstract: A clipping system includes an adapter configured to be mounted over an insertion device and a clip including first and second jaws movable between an insertion configuration and an initial deployed configuration. A first extending member is releasably coupled to the clip and movably connected to the adapter to permit a movement of the clip relative to the adapter from the insertion configuration to the initial deployed configuration. The first extending member is configured to permit withdrawal of the adapter away from the clip to place the system in a review configuration to enhance a visual observation of the clip. The first extending member is operable to retract the clip over the adapter so that the clip is forced open, freeing the clip from tissue on which it has been clipped. The first extending member is configured to release the clip therefrom in a final deployed configuration.

The support mechanism may also include an instrument holder that extends along a central axis. The instrument holder may be movably coupled to the bracket and adapted to be coupled to a second medical device. The bracket and the instrument holder may be oriented such that central axis of the instrument holder intersects the longitudinal axis at an angle.

Abstract: A clipping system includes a cap configured to be mounted over a distal end of an endoscope. A clip mountable over the cap to move between an insertion configuration, in which first and second jaws extend over opposing portions of the cap so that the first and second jaws are separated from one another, a review configuration, in which the clip is moved distally relative to the cap so that a portion of the clip extends distally until at least a portion of the clip extends into a field of view of the optical system of an endoscope on which the cap is mounted, and a deployed configuration in which the clip is moved distally off of the clip so that the first and second jaws are drawn toward one another under the bias of the at least one hinge to close over tissue received therebetween.

Abstract: According to one aspect, a tissue fastening device may comprise a body configured to include a plurality of fasteners; an anvil rotatable relative to the body; and a tissue cutting device. The body may define a longitudinal channel configured to receive the tissue cutting device. The tissue fastening device may further comprise a fastener actuator configured to move proximally relative to the body to deploy the plurality of fasteners from the body; and an actuation wire coupled to the tissue cutting device and the fastener actuator. The actuation of the actuation wire may cause the fastener actuator to deploy at least one of the plurality of fasteners into tissue distally of tissue cut by the tissue cutting device.

Abstract: Improved methods and devices for performing an endoscopic surgery are provided. Systems are taught for operatively treating gastrointestinal disorders endoscopically in a stable, yet dynamic operative environment, and in a minimally invasive manner. Such systems include, for example, an endoscopic surgical suite. The surgical suite can have a reversibly expandable retractor that expands to provide a stable, operative environment within a subject. The expansion can be selectively asymmetric to maximize space for a tool and an endoscope to visualize a target tissue and treat the target tissue from outside the patient in a minimally invasive manner.
