Abstract: Disclosed herein is a telescoping bipolar electrode (1) having an inner electrode (4) with an outer diameter (D4), length and outer side surface, such that up to the entire side surface is covered with an insulation sheath (5), which alternatively is fixed to the inner electrode (4) or is lengthwise adjustable with respect thereto, the inner electrode (4) and insulation sheath (5) together being slidably mounted within the inner space of an outer electrode (2) having an inner diameter greater than that of the inner electrode and its insulation sheath, an outer diameter (D2), thickness, length and an outer side surface area, the outer electrode (2) also having an insulation sheath (3) around its outer diameter (D2), covering up to the entire side surface area of the outer electrode (2), with the insulation sheath (3) around the outer electrode (2) being alternatively fixed to the outer electrode (2) or lengthwise adjustable with respect thereto. The bipolar electrode (1) is capable of being inserted into the human body through a body cavity or through the lumen of a vessel. At least one of the outer and inner electrodes is supplied with radio frequency energy from an external source (10). The bipolar electrode (1) is utilized to cut, desiccate or ablate neoplasms (12) in the body. In an alternative embodiment, the bipolar electrode (1) is configured such that the two component electrodes (2, 4) are arranged externally parallel to one another and are telescopically slidable relative to one another.

Abstract: An apparatus having a vibratable tip (22) for ultrasonically disintegrating tissue in a surgical procedure and for aspirating the disintegrated tissue and fluid away from the surgical site (12) through an opening (24) in the tip. A connection (42) to the electrosurgical unit provides for delivery of RF cutting current, RF coagulating current or a blend thereof to the tip so that electrosurgical procedures can be conducted separately or simultaneously with ultrasonic aspiration through the tip. It thus is now possible, for example, to electrocauterize simultaneously with ultrasonic fragmentation with a single handheld surgical device. The simultaneous delivery of ultrasonic vibrations and RF current to the tip also has been found to increase the tissue fragementation rate.

Abstract: An ultrasonic and electrosurgical handpiece housing has an axis, a distal end, a proximal end and a passage defined by an inside wall. A piezo stack has elements carried along a plane generally transverse to the axis. A retainer for the elements has distal, intermediate and proximal portions. The distal portion has a fastener; the proximal portion has a threaded shoulder bolt. The stack about the shoulder has an energy input and a reference output positioned axially apart from one another. An insulating sleeve about the bolt is between central openings of the elements and the bolt. A pair of insulators about the bolt has the stack therebetween; one insulator is between the plane and the stack and the other is near the threads. The intermediate portion attaches to the housing at the plane with decoupling and sealing. An irrigation channel conveys fluid. The drive energy input, the feedback signal and the reference outputs are insulated by the sleeve and pair from the bolt. A washer against the insulator and about the threads applies axial but not torsion to the insulator when a clamping nut on the threads holds components on the bolt. The clamping nut is in circuit with electrosurgical energy as the reference return for electrosurgery. A tool fastens to a connector for transmitting axial vibrations in the form of a predefined stroke at a tip thereof.

Abstract: A minimally invasive retractor and dissector (10) for internal surgical use on a patient has a tubular support (11) for passing into the body; the tubular support (11) has a passage (16) for access along an axis "A" thereof during operative procedures on the patient's internal tissue. A proximal end (12) on the tubular support (11) is located outside the patient in position to be accessed by the surgeon when a distal end (13) is inside. One or more jointed articulated members (15) are movably positioned and capable of holding and pulling tissue disposed beyond the distal end (13) and each member has a distal tip (17) to hold tissue. An instrument capable of moving independent of any member functions cooperatively on the tissue and passes through the passage (16) from the proximal end (12) to beyond the distal end (13) as the members position the tissue. The instrument is a surgical tool and the tissue is maneuvered by the members within the body into a position relative to the tool. A control (19) at the proximal end (12) manipulates the members relative to the axis "A" permitting holding and pulling tissue disposed beyond the distal end (13); the control (19) is capable of moving the instrument independent of the members for operating on the held and maneuvered tissue. A grip (34) manipulates the members and positions the instrument. A method for using the minimally invasive retractor and dissector (10) has the steps of making an opening for a tubular support (11) to pass into the body, leaving the proximal end (12) on the tubular support (11) outside the body, positioning the distal end (13) thereof inside, and moving members carried on the tubular support (11). Holding and pulling tissue disposed beyond the distal end (13) and using the passage (16) for access along the axis thereof for operating on internal tissue are added steps. Another step is moving an instrument in the passage (16) independent of the members.
