Abstract: PROBLEM TO BE SOLVED: To provide an electrode and a nozzle combination to transport ionizable gas to the end of gas-reinforced electric surgery equipment. SOLUTION: An electrode 32 and a nozzle combination 30 have the base end part 34, the tip end part, and a part with a constant length between the base end part 34 and the tip end part 33, and transport an ionizable gas to an electric surgical pencil set on a patient. A shroud 36 surrounds the electrode 32 along the part with a constant length, but the base end part 34 and the tip end part 33 are made to remain exposed. A port 41 including a tube cavity communicating with a passage 39 is formed on the shroud 36 to make a fluid capable of communicating between the passage and the tube cavity. An end cap 43 is mounted on the base end part 38 to form an airtight seal between the base end part of the electrode and the rear part 38 of the shroud 36.

A piezo stack has elements carried along a plane generally transverse to the axis. A retainer for the elements has distal, intermediate and proximal portions. The distal portion has a fastener; the proximal portion has a threaded shoulder bolt. The stack about the shoulder has an energy input and a reference output positioned axially apart from one another. An insulating sleeve about the bolt is between central openings of the elements and the bolt. A pair of insulators about the bolt has the stack therebetween; one insulator is between the plane and the stack and the other is near the threads. The intermediate portion attaches to the housing at the plane with decoupling and sealing. An irrigation channel conveys fluid. The drive energy input, the feedback signal and the reference outputs are insulated by the sleeve and pair from the bolt. A washer against the insulator and about the threads applies axial but not torsion to the insulator when a clamping nut on the threads holds components on the bolt. The clamping nut is in circuit with electrosurgical energy as the reference return for electrosurgery. A tool fastens to a connector for transmitting axial vibrations in the form of a predefined stroke at a tip thereof.

Abstract: A connection of first and second members wherein each has a body and a connecting end with an inner surface so the second connecting end has an outer surface shaped for telescoping into the inner surface of the first connecting end. A shoulder is located on the second member and a groove is in the outer surface of the second connecting end. A raised nub on the inner surface of the first connecting end is radially inward to engage the groove. An elastomeric gasket is positioned to bear against the shoulder and be compressed between the first and second bodies. An engaging trough in the groove holds the first connecting end against the shoulder with the nub against the groove by the compression force of the gasket. An indexing trough in the groove positioned along the groove beyond the engaging trough allows the nub to follow therethrough and slightly compress the gasket thus giving the user a tactile indication that it has been rotated beyond the engaging trough. A method for fluid tight attachment of members of a medical device has steps including providing the connection of the first and second members with ends by telescoping the ends. Locating a groove for operating the groove as a cam and locating a raised nub of material on the inner surface of the first connecting end for positioning the raised nub to engage the groove for operating the raised nub as a follower of the cam are steps. Compressing a gasket seal between the first connecting end and the shoulder while making the gasket seal from an elastomeric material is a step. Positioning an engaging trough in the groove to hold the first and second connecting ends against each other is a step. Positioning an indexing trough in the groove so when the nub is rotatably forced past the engaging trough it will further compress the first end against the gasket is a step.

Abstract: An ultrasonic surgical handpiece for surgery on a patient has a handpiece with distal and proximal ends on a housing thereof for manipulation by a surgeon. A vibrator is carried within a hollow inside of the housing. A tool extends from the vibrator at the distal end to a distal tip and is tubular with a bore along its axis so axial vibrations at high frequency are transmitted to the distal tip. Fluid flows from the distal tip to the vibrator, cools the vibrator and connects to a source of suction at the proximal end. A flue carried cantilever on the distal end extends about the tool so a space is between the tool and the flue for guiding fluid from the distal end to the distal tip. A port located through the tool connects between the space and the bore and is axially positioned near the distal end to collect fluid. An inlet on the flue is near the distal end for fluid provided by a supply. The inlet is axial distal of the port and/or may meter fluid to a preset range. The port may be at, near or toward a low stress region of the tool. The vibrator is piezo. The port can be diametrically opposed to the inlet. Holes may be located through the tool near the distal tip to prevent misting, clogging or dripping of fluid. A method of cooling the ultrasonic surgical handpiece has steps that circulate fluid from the inlet to the space and port and pass cooling fluid about the vibrator and to the source of suction. A method of making the ultrasonic tool may include having one or more ports through the tool to the bore wherein the ports are at, near or toward low stress regions and allow fluid flow from the space to the bore.

The drive energy input (29), the feedback signal and the reference outputs (30) are insulated by the sleeve (31) and pair from the bolt. A washer (34) against the insulator (32) and about the threads applies axial but not torsion to the insulator (32) when a clamping nut (35) on the threads holds components on the bolt. The clamping nut (35) is in circuit with electrosurgical energy as the reference return for electrosurgery. A tool (36) fastens to a connector (37) for transmitting axial vibrations in the form of a predefined stroke at a tip (38) thereof.
