Abstract: There is provided a system of steering stimulus pulses to one or more selected cardiac sites, for improved capture and for easy adjustment of the stimulus pulse when threshold level has changed, without the need to simply increase stimulus power or to replace an implanted lead. The invention utilizes, in a cardiac environment, the technique of delivering substantially concurrent pulses across two or more electrode pairs, each component pulse being adjusted as to pulse, duration and/or phase to steer, or direct the resultant composite pulse toward the target cardiac site. Embodiments of the invention include steering pulses for pacing of the left atrium; steering pulses for multichamber pacing; improved use of a single lead system with electrodes floating in the atrium, with specific steering of atrial pulses to provide an enhancement of a VDD system; steered delivery of atrial and/or ventricular defibrillation stimuli; and steered stimuli for arrhythmia prevention. In other specific embodiments, the steered pulse system is combined with capture detection and automatic adjustment of the steering parameters.

Abstract: Apparatus and methods are disclosed for reducing the potentially harmful effects of electromagnetic waves on an implantable medical device. In one embodiment of the present invention, an implantable medical system comprising a dual threshold magnetic sensor capable of detecting an elevated magnetic field is disclosed. The sensor can comprise a solid-state sensor capable of detecting a static magnetic field in excess of about 1500 Gauss. The magnetic sensor can be operatively coupled to electronics that are capable of altering the operation of the system upon detection of an elevated magnetic field by the magnetic sensor.

Abstract: Electrical tissue stimulator and method for limiting the programmability of stimulation pulse parameters. The tissue stimulator includes a plurality of electrodes and a memory for holding a stimulation pulse parameter limit value and a programmed parameter value of a stimulation pulse. A pulse generator applies electrical stimulation pulses to the electrodes in accordance with the programmed parameter value. Programming means is included for receiving remotely generated programming signals and altering the contents of the memory in accordance with the signals. Means is included for comparing the stimulation pulse parameter limit value and the programmed parameter value and for preventing and alteration of the programmed parameter value, unless it is within stimulation pulse parameter limits. The stimulation pulse parameters may include pulse amplitude, pulse width, and pulse rate and the programmed parameter limit values may be upper and lower limits.

Abstract: A patient programmer for facilitating patient control over an implanted medical device, such as an implanted spinal cord stimulator or the like, is disclosed. A programmer in accordance with the present invention is a portable, light-weight, easy to use patient programmer. The programmer enables a patient to control the operation of his or her implanted device outside of a clinical setting. The programmer incorporates features which ensure that users of varying ages, education levels, dexterity levels, physical and mental health will be able to safely control the operation of their implanted devices, within predefined limits established by a physician or clinician. Circuitry is provided for avoiding battery depletion and/or undesired programmer/implanted device communication in the event that the programmer's key is accidentally depressed (for example, due to being jammed into a purse or pocket) or is depressed repeatedly or continuously over an extended period of time. The programmer provides tactile, audible, and visible feedback to the user to convey information regarding the proper (or improper) operation of the programmer and the implanted device. In one embodiment, the device includes a beeper and light-emitting diodes (LEDs) to indicate, for example, that a desired programming function has been successful or unsuccessful.

Abstract: A medical trial neuro-stimulator has a diagnostics module to determine if a stimulus therapy link is correctly set for the patient. The trials neuro-stimulator is used e.g. to test the efficiency of the neuro-stimulation before a neuro-stimulator implant is introduced by surgery. The trial neuro-stimulator has a processor, a memory-storage component, a system-reset, a telemetry module, a charge module, an energy management module, an energy source, a therapy module, a therapy measuring module and a diagnostic module. The diagnostic module may incorporate a link sensor, software detector, therapy module link measurement, or a combination of the link sensor and software detector. Also claimed is a diagnostic start-up procedure.

Abstract: An array of stimulation electrodes (AS1) includes 55 electrodes such as flat insulated electrodes (10-12) arranged in a rectangular grid and the top surface of the array is exposed to patient tissue at the surface of lead (L1). A controller (C1) is connected to a conductor (ID1) to supply data from a data source (D1), while stimulation current is passed to the array via conductors (P1,P2). The data source may be a microprocessor with a memory storing data identifying electrodes to be activated and their polarities. AN Independent claim is included for a method of interacting with electrically excitable tissue of a patient.

Abstract: The technique involves using a main cable for electrical interaction with a patient. A matrix of electrodes (AS1) on at least one implantable tip (L1) is implanted to interact with the patient's tissue. A cable (CB1) with fewer conductors (P1,P2) than the number of electrodes is laid between a remote point and a point near the tissue. At least one electrode is selected and connected to the main cable to form an anode and cathode relationship between at least one pair of non-adjacent electrodes for signal transfer to the tissue with a minimum of conductors. An Independent claim is also included for an arrangement of interacting with patient tissue that can be electrically stimulated.
