Abstract: An apparatus and method for igniting plasma in a surgical system is disclosed. A corona discharge is generated on a surgical handpiece which is used to ignite a plasma arc (19) for surgical operations. The advantages include greater reliability and repeatability of plasma arc (19) ignition. The apparatus comprises a handpiece incorporating an active electrode, a passage for ionizable gas (12), and a corona return electrode (17). The corona return electrode (17) has a terminus on the holder (11) and near the distal end (20) of the holder (11). The corona return electrode (17) is electrically connected to the return path of the electrosurgical generator. A non-uniform electric field is generated between the active electrode (14) and the corona return electrode (17) of sufficient strength so that a corona is formed near the active electrode (14). A separate return electrode (17) may be on the patient (25), or the apparatus may be configured for bipolar electrosurgical operation by carrying the return electrode (17) on the handpiece. A dielectric material separates the active electrode (14) and the corona return electrode (17). There is substantially capacitive coupling between the active electrode (14) and the corona return electrode (17). There is substantially resistive coupling between the active electrode (14) and the return electrode (17).

Abstract: A device for retrograde hole opening through tissue has a member elongate on an axis with a cross section shaped to insert axially through external tissue. A distal and a proximal end on the member respectively enter the tissue during placement and remain outside the tissue for control. A tip at the distal end has a deployable tissue divider with one or more tissue parting elements and each has a splitter. The tissue parting elements are located within the cross sectional dimensions of the member in a storage position and are movable relative to the tip for placement in an exposed position relative to the tip when shifted from storage so that the splitter thereof splits tissue during retrograde extraction along the axis and contact with tissue. Linkage between the proximal end and the deployable tissue divider retains each of the tissue parting elements with its splitter exposed. The deployable tissue divider has an electrode for transmitting radio frequency energy received from the proximal end to at least each splitter. A return path completes the circuit to provide an electrosurgical effect during the retrograde extraction. A method of placing the device for retrograde hole opening aligns the axis of the elongate member normal to the outside abdominal wall of the body, places the distal end through the tissue and leaving the proximal end outside of the tissue, deploys the tissue divider having one or more tissue parting elements that are first located in a storage position within the cross section to an exposed position with the splitter of each element positioned to split tissue, moves each element axially while exposed and fixed and splits tissue during extraction from the body cavity.

Abstract: An actuator for a self-adjusting pump head assembly has a variable position pump shoe slidably attached to a base. The assembly pumps liquids through a tube in a peristaltic pump, including a translator of rotational motion into linear motion and a crank for automatically compensating for the manufacturing tolerances of the tube in the pump pivotally attached to the translator and the shoe. The crank for automatically compensating has a linkage or pair of links, carrying a helical compression spring, pivotally anchored to the translator and the shoe. The peristaltic pump includes a self-adjusting pump head, including a variable position pump shoe slidably attached to a base and a control for positioning, locking and applying a continuous reaction force on the shoe to compress the tube between the shoe and at least one roller located on the periphery of a mandrel. Thus the control further has the translator of rotational motion into linear motion, and the crank for automatically compensating for the manufacturing tolerances of the tube introduced into the pump, pivotally attached to the translator and the shoe. Alternatively the pump has a disposable manifold safety cartridge, removably attached to the base, to which ends of the tube attach. The cartridge has an asymmetrical tie bar keyed onto the base to insure that the cartridge is oriented in an acceptable manner and that the tube will be properly installed on the pump.

Abstract: An ultrasonic and electrosurgical handpiece housing has an axis, a distal end, a proximal end and a passage defined by an inside wall. A piezo stack has elements carried along a plane generally transverse to the axis. A retainer for the elements has distal, intermediate and proximal portions. The distal portion has a fastener; the proximal portion has a threaded shoulder bolt. The stack about the shoulder has an energy input and a reference output positioned axially apart from one another. An insulating sleeve about the bolt is between central openings of the elements and the bolt. A pair of insulators about the bolt has the stack therebetween; one insulator is between the plane and the stack and the other is near the threads. The intermediate portion attaches to the housing at the plane with decoupling and sealing. An irrigation channel conveys fluid. The drive energy input, the feedback signal and the reference outputs are insulated by the sleeve and pair from the bolt. A washer against the insulator and about the threads applies axial but not torsion to the insulator when a clamping nut on the threads holds components on the bolt. The clamping nut is in circuit with electrosurgical energy as the reference return for electrosurgery. A tool fastens to a connector for transmitting axial vibrations in the form of a predefined stroke at a tip thereof.

Abstract: An electrosurgical tip for the application of electromagnetic energy in either a monopolar or a bipolar circuit through the tissue and the bodily fluids of an animal or human has a metallic electrically and thermally conductive electrode for connection to an electrosurgical generator proximally and for transmission of the electromagnetic electrosurgical energy to the tissue and the bodily fluids of the animal or human distally. A sheath of heat shrinkable fluorinated polymeric substance is an electrical and thermal insulator held about the electrode for preventing the flow of electromagnetic and thermal energy from the metallic electrically and thermally conductive electrode to the tissue and the bodily fluids. A smooth surface of the sheath contacts the tissue and the bodily fluids and has a low surface free energy. Openings through the sheath extend from the electrode to the surface for permitting the passage of energy. A wall of the sheath is of a thickness adequate to space the electrode from substantially any contact with the tissue and isolate heat due to the arcs. A ceramic substance is an electrical and thermal insulator about the conductive electrode for preventing the energy flow. A method of making the tip includes covering the electrode with the insulator, isolating physically the heat generated due to the passage of energy. A method of using the tip places the surface in close proximity to the tissue and the bodily fluids, transmits the electromagnetic energy through the openings, allows cleaning tissue and bodily fluids from the surface. The use includes first passing energy through openings in a wall of electrical and thermal insulation and application of electromagnetic energy by either a monopolar or a bipolar circuit relative to the tissue and the bodily fluids of an animal or human.
