The present disclosure relates to a method for preparing a haemostatic composition comprising thrombin, the method comprising the step of reconstituting a dry thrombin directly in a paste, such as a paste comprising a biocompatible polymer. The haemostatic composition comprising thrombin may be prepared from a dry thrombin composition and a paste in a single step operation and be used for treatment of a wound.

A method of making a biocompatible composition for sealing tissue includes mixing a first fluid having a first reactive component (e.g., an electrophile) and a second fluid having a second reactive component (e.g., a nucleophile) to form a mixture and expressing the mixture. During expression, a pH modifying fluid (e.g., NaOH) is added to the mixture at a rate that changes. In one embodiment, a higher ratio of the pH modifying fluid is added to the mixture during a first expressing stage and a lower ratio of said pH modifying fluid is added to the mixture during a second expressing stage. During the first expressing stage, a mixing ratio of the pH modifying fluid, the first fluid and the second fluid is 0.7-1.4:1:1. During the second expressing stage, the mixing ratio of the pH modifying fluid, the first fluid and the second fluid is 0.12-0.24:1:1.

An applicator instrument for dispensing surgical fasteners includes an elongated shaft, a housing connected with the elongated shaft, a trigger, a gear train, and a firing system disposed in the housing. The trigger is squeezed for commencing a firing cycle. The firing system includes a storage reel, a spool connected to the storage reel, a drive wheel located distal to the storage reel and the spool, a constant torque spring having a proximal end connected to the spool and a distal end connected to the drive wheel, and a flexible member in contact with the drive wheel. The flexible member has a proximal end connected to the storage reel and a length configured to be driven by the drive wheel toward the distal end of the elongated shaft. During an energy storing stage, the gear train is coupled with the storage reel for rotating the storage reel in a counterclockwise direction, which, in turn, retracts and winds the flexible member onto the storage reel, which rotates the drive wheel in a counterclockwise direction and winds the constant torque spring from the spool onto the drive wheel for storing energy in the constant torque spring.

An applicator instrument for dispensing surgical fasteners includes a housing, an elongated shaft extending from the housing, a firing rod disposed inside the elongated shaft and being movable within a first plane between a retracted position and an extended position, an advancer disposed inside the elongated shaft and being moveable within a second plane between a retracted position and an extended position, and a staging assembly located adjacent the distal end of the elongated shaft and being adapted to align surgical fasteners with a distal end of the firing rod. The staging assembly is held below the second plane by the advancer when the advancer is in the extended position and the staging assembly is adapted to move into at least partial alignment with the distal end of the firing rod when the advancer is in the retracted position.

A surgical fastener includes a first leg having a first distal barb and a first proximal barb that extend inwardly, and a second leg having a second distal barb and a second proximal barb that extend inwardly. A bridge interconnects proximal ends of the first and second legs. A first distance between opposing inner tips of the first and second distal barbs is greater than a second distance between opposing inner tips of the first and second proximal barbs. The first leg tapers inwardly between the proximal and distal ends thereof and has a cross-sectional area that is greater at the proximal end than at the distal end. The second leg tapers inwardly between the proximal and distal ends thereof and has a cross-sectional area that is greater at the proximal end than at the distal end. An insertion tool engages a crown on the proximal side of the bridge.
Abstract:
Mesh implant positioning systems for use in laparoscopic body wall defect repairs, such as hernia defects, are disclosed. The systems utilize one or more bar members combined with surgical sutures and needles to position a mesh implant adjacent to a body wall defect without the need for stay sutures. Also disclosed are assemblies of the positioning systems with surgical mesh repair implants, and methods of repairing body wall defects such as hernias with the assemblies.

The distal end cap has a surgical fastener dispensing window for dispensing the one or more surgical fasteners from the distal end cap. The firing system includes a firing rod that extends through the elongated shaft for dispensing the one or more surgical fasteners from the distal end cap. The firing rod has a proximal section that moves along the first axis of the elongated shaft and a distal section that moves along a second axis that defines an angle with the first axis.

An applicator instrument for dispensing surgical fasteners includes a housing, a firing system disposed in the housing and moveable in distal and proximal directions along a first axis, a handle extending upwardly from the housing along a second axis that defines an acute angle with the first axis, and a trigger mounted on the handle. The applicator instrument includes a non-linear elongated shaft extending from the housing that has a distal section that is extends upwardly toward a top of said applicator instrument. A distal end cap is secured to the distal end of the elongated shaft. The applicator has an imaging system including an imaging device for detecting images at the distal end of said applicator instrument, and a light source for illuminating a field of view for the imaging device at the distal end of the applicator instrument.
