Disclosed herein are methods and compositions for increasing and decreasing the permeability of the blood brain barrier for the treatment of diseases and conditions and to facilitate the delivery of agents to the brain, as well as methods and compositions for promoting re-myelination and preventing de-myelination. Compositions include RGMa, soluble RGMa, and functional fragments and variants thereof, RGMc, soluble RGMc, and functional fragments and variants thereof, and Neogenin peptides including 4Ig.

Disclosed herein is an agent that modulates a cis interaction between Repulsive Guidance Molecule A (RGMa) and Neogenin or lipid rafts. Modulation by the agent may include blocking the cis interaction between RGMa and Neogenin and/or disrupting lipid rafts. In turn, this promotes neuronal cell survival and axon growth and/or regeneration. Also disclosed herein is a method of treating a disease in a subject in need thereof. The method may include administering the agent to the subject. Further disclosed herein is a method of identifying an agent that modulates the cis interaction between RGMa and Neogenin.

Methods of stimulating neuron survival, axon growth and/or promoting axon regeneration of a neogenin expressing cell in a subject in need thereof are provided. The methods involve for example, administering to a subject in need thereof a PPC inhibitor, optionally a SKI-1 inhibitor and/or a furin inhibitor. Cleaved RGMa inhibits neogenin mediated axon growth. The SKI-1 inhibitor and/or furin inhibitor inhibit cleavage of RGMa, thereby stimulating neuron survival, axon growth and/or promoting axon regeneration.
